Professional Outreaching Team for Private Residential Care Homes for Persons with Disabilities (POT)

Description

To provide multi-disciplinary outreach services for residents of private residential care homes for persons with disabilities (including persons with physical and / or intellectual disabilities, persons in mental recovery or persons with suspected mental health problems, etc.) so as to meet their social and rehabilitation needs.

Residents of private residential care homes for persons with disabilities (include those participate in the "Bought Place Scheme for Private Residential Care Homes for Persons with Disabilities")	Outreach services by physiotherapists, occupational therapists, speech therapists, registered nurses (psychiatric) and social workers; Conduct assessments, formulate rehabilitation plans, provide individual or group rehabilitation trainings, treatments and regular review on the rehabilitation progress for the residents by therapists; Provide nursing related services (including consultations on emotion / behavioral management, health management trainings and drug management, etc.) by registered nurses (psychiatric); Provide counseling services and organise various social and recreational activities, make referrals for other appropriate services for the residents when necessary, as well as establish linkage with the community for the residents and facilitate their integration into the community through community networking activities by social workers; and Provide assessments and psychological counseling by clinical psychologists.
