Transitional Care and Support Centre for Tetraplegic Patients - Day Service
Description
Transitional Care and Support Centre for Tetraplegic Patients (TCSC) provides time-defined and goal-oriented community-based rehabilitation programmes to tetraplegic patients discharged from hospital/medical rehabilitation centre to facilitate their return to community living with a view to improving their physical, cognitive, communicative, behavioural, psychological and social functioning in a non-medical setting. Training and support services will be provided to family members or carers of the patients to strengthen their caring capacity.
Target Group
- Discharged patients suffering from spinal cord injury, neuro-muscular disease or severe spastic resulting in tetraplegia, who are aged 15 or above;
- Family members / carers of the above discharged patients.
Nature of Services
The services provided include:
- Transitional Residential Service;
- Ambulatory Day Training Programme;
- Support Services including residential respite care service.
Application Procedure
Transitional Residential Service and Ambulatory Day Training Programme
- Referred by medical officers or allied health professionals of hospitals or special clinics under Hospital Authority. The application is free of charge.
Support Services
- Applicants can approach the centre for direct application for the service. The application is free of charge.
Fees and Charges
- Transitional Residential Service: $1,871 per month;
- Residential Respite Care Service: $81 per day;
- Ambulatory Day Training: $61 per session.
