The Pilot Project on Integrated Community Rehabilitation Centre (ICRC)
Description
The Social Welfare Department has sought from the Lotteries Fund to implement “The Pilot Project on Integrated Community Rehabilitation Centre” (the Pilot Project) through 2 ICRCs to provide a continuum of coherent and interconnected community support services for persons with disabilities having intensive care needs and living at home and to provide support to their carers. The ICRCs set up under the Pilot Project will provide integrated day care and home-based support services to be delivered in a flexible manner which will cater for the actual impairment level and changing needs of the persons with severe disabilities.
Target Service Users
The target service users (with a limited number of persons with moderate disabilities) are the persons with severe disabilities aged at 15 or above living in the community -
- suffering from severe disabilities (intellectual and/ or physical), stroke, neurological or physical impairment; OR
- referred by medical officers/occupational therapist /physiotherapist or assessed by the ICRC as suitable for the Services (including persons with moderate disabilities).
Scope of Service
The core service components of the services of the ICRCs shall include integrated case management with ongoing assessments according to the service user's changing in health condition, body functions, his/her need for activities, environmental factor and personal factor so that following these components, to adjust the intensity of services for individual service user (i.e. centre-based or home-based). In addition, the ICRCs are required to:
- Both Centre-based and Home-based Services1
- nursing care;
- physiotherapy;
- occupational therapy;
- speech therapy;
- tele-rehabilitation training;
- personal care;
- community and living skills training; and
- respite service (including emergency day respite service).
- Home-based Services
- escort;
- delivery of goods; and
- home-based training.
- Centre-based Services
- sensory integration training;
- multi-sensory therapy; and
- reminiscence therapy.
- Application of International Classification of Functioning, Disability and Health (ICF) framework
- explore applying the ICF framework in devising a structure (e.g. service framework and a package of centre-based and home-based training for different Service Users) comprising rehabilitation objectives, intervention, assessment and evaluation. The case managers of ICRCs should formulate an individual care plan (ICP) for each service user after consulting the multi-disciplinary professionals by using ICF; and
- facilitate data and experience sharing among rehabilitation organisations.
- Application and Promotion of Assistive Technology
- use technology products/applications2 and promote assistive technology in the community through renting the assistive technology items suitable to be used at home;
- conduct home modification; and
- collaborate with the business sector, medical sector, special schools, academic institutions and other NGOs equipped with the knowledge of application of innovative technology products for exploring the feasibility of promoting the application of assistive technology in the community.
- Support for Family members/Carers
- need assessment for Carers;
- supportive counselling and group work for Carers; and
- training for enhancing care skill.

Service charges are based on different nature of services and activities provided.
