Personal Data Access Request Form

• For access to personal data in accordance with the Personal Data (Privacy) Ordinance ('PD(P)O'), please complete the Data Access Request Form which is specified by the Privacy Commissioner for Personal Data.
• For obtaining photocopy of personal data in accordance with PD(P)O, a photocopy charge set by the Treasury will be levied. The standard photocopying charge (black and white copy) is HK$1.50 per photocopy on A4 size paper and HK$1.60 per photocopy on A3 size paper. Photocopying made on both sides of a sheet is counted as two copies.
